Lombardy & Milan
City life, top-tier care.

Milan is the Italy that runs on time: the country's most international city, its best hospitals, and a cultural calendar that never hibernates. The lake towns an hour north have their own page — this one is about city life. Around Milan sit Lombardy's provincial cities — Bergamo, Brescia, Mantua, Pavia — offering the same healthcare region at gentler prices.
Towns retirees shortlist
Milan itself suits retirees who want walkable neighborhoods, museums, and specialists ten minutes away — and who can afford it. Bergamo and Brescia give you real city services at noticeably lower prices; Mantua and Pavia are smaller, quieter, and university-civilized. All of them sit inside Lombardy's health system, which is the point.
What it costs, honestly
- Milan is Italy's most expensive housing market — average asking prices run above €4,000 per square metre, more than double the national average, and central one-bed rents commonly run €1,200–1,800. We will not pretend otherwise.
- Provincial Lombardy is a different story: Bergamo, Brescia, and Mantua price well below Milan while keeping the same regional services.
- Lombardy does not qualify for the 7% foreign-pensioner flat-tax regime. Plan on ordinary Italian tax rates or treaty treatment.
Healthcare access
The strongest card in this pattern, and arguably in Italy: Lombardy's regional health system is regarded as the country's best, Milan's hospitals and specialists are top-tier, and English-speaking doctors are easier to find here than anywhere else in Italy. If healthcare access outranks cost for you, this is the honest answer.
Day to day
Northern efficiency, Italian paperwork: services work, public transport is genuinely good, and Milan is the one place on our list where life without a car is easy. Comune offices still operate in Italian, on weekday mornings, at their own pace — some things no region exempts you from. Winters are grey and foggy; summers hot and humid.
Who this fits
Retirees who want city culture and Italy's best healthcare at the door, who have the budget for it, and who prefer four real seasons to the beach.
Worth knowing: The Po Valley climate is the honest trade: Milan winters are foggy and gloomy for weeks, and summers are hot and humid. Come in January before you sign anything — if the grey doesn't bother you, everything else here works.
